Resources from the Diocese:
Begin by going to www.formed.org, and selecting the options to "Sign Up," and "I belong to a Parish or Organization." After this, select All Saints Parish in Le Mars and enter your information. It will have you check your email account to verify, and then you should be able to log in.
You will notice that it does not require the use of a password, as it secures the accounts through email verification. It seems to be easiest to create an account on a computer, even if you will be downloading the app onto a Roku, smartphone, or other device.
